Would you like free training for 2-4 clinic staff to help improve health outcomes for patients struggling with diabetes self-management? 


This half-day training on May 9 will equip front-line healthcare professionals with essential skills to address social determinants of health affecting diabetic patients.

Register by April 24 to secure your spot, as seats are limited. 


Training overview: 

Purpose: To empower healthcare professionals in Johnson County to identify and effectively support patients facing barriers that result from social determinants of health barriers, therefore improving patient health outcomes and care metrics.


Cost: Free to participating clinics. This training has been underwritten by and developed in partnership with the Johnson County Department of Health and Environment.


Limited seats are available for this interactive training.

On May 9, your front-line healthcare professionals who screen diabetic patients for, or assist with, social determinants of health could attend a half-day training to build skills in:

  • Understanding how social determinants of health affect the health of people with diabetes.
  • Identifying seven common needs and the barriers they create for diabetic people.
  • Learning the healthcare professional's role in addressing and screening for SDOH.
  • Practicing interpersonal skills to build confidence and understand sensitivities in screening for and discussing SDOH.
  • Demonstrating best practices to assess and implement SDOH screening tools.
  • Connecting patients to resources that can address their specific challenges.

 

This program is ideal for staff of Johnson County clinics who:

  • Conduct initial screenings of patient needs, including your SDOH screening;
  • Review and follow up with that screening;
  • Provide RN case management;
  • Serve as Community Health Workers; or
  • Otherwise work directly with patients with diabetes to improve their capacity to effectively manage their condition.
